How can i remove FB EXTRAS from my FB account in windows 7?

i installed it from here http://www.fbskins.com/facebook-layouts/
it shows 'fb extras' and 'select new skin' at top of left side on my fb profile.
and whole left side is blocked by advertisements.
please help me

press [shift & ESC] at the same time a window should pop up then click on extension fb layout then at the bottom of the window click end task then close the window and close your facebook then reopen your facebook and it should be gone!
