Ho do I turn off an app on Facebook? Read more below...?

So I saw this app on Facebook called "What animal were you in past life?" and used it. It's really annoying and I don't know how to turn it off. How?

Click the gear icon in the upper right corner of your Facebook page. Choose Account Settings. Click Apps on the left side. Find the app you want to remove, and click the [x] to the right of it.

just go to the main page (click the facebook "logo" top left corner) then click on apps and you will see a list of the apps you have used look for the one you want to remove and click on the little pen to the left of the name and chose remove app .
