I received an Instagram Direct?

I received an instagram direct and this person sent the same picture to a number of people. If you are the first person in the "sent to" icons (left to right) does this mean that you are the first person this person chose to send it to, or does it mean you're the first to receive it, maybe it doesn't mean anything? Thank you.
