It's automated by the game itself.

It's because the game that person happens to play spam all his contacts with game request. It seems to be common for many Facebook games

I'm pretty sure there's a checkmark box or something that says "send request" or something like that (I'm not on facebook right now, so I'm not entirely sure. Also, it could be different depending on the game). But if you don't uncheck that box or click "cancel", then facebook will send game requests. So I think it's the person's fault because they don't bother to change that.
