Go to the Account tab at the rightmost corner. Then go to account settings. From the list on the top left, select 'Applications'. This lists all your apps and you can remove or edit them. About the first part of the question, I have not seen that yet. May be its one of facebook's patch releases and will come to all of us eventually.
