each album has a "high quality" check box at the bottom - make sure that is enabled before uploading a picture to that album

Just because your image size is not really suit for Facebook. If it‘s too big or small, the banner will be not clear. The banner size of Faceboook cover is 850*315, Here is a tutorial for you:
