Send your query to the admin on the following page:

Specify why you think the page needs to be shut down and make sure you add the URL too.

type "how do i permanently delete my facebook account" in google and click the result from facebook help center.

on the explanation page, there is a linked word "deactivate" in blue and you can follow the instruction from there
