how to add form at my site?

Hi Yahoo,
I just signed up for the the Yahoo Small Business (Advance package) web hosting. May i know how do i add form at my site? i have gone to your "help" site about adding form topic, but what you shown at the task bar is totally different from mine.....your site builder comes with "insert" at the top task bar while mine doesn't.
Please help ASAP, thanks!

You can use the google apps to create the form.
