The first step is to get a host.

Next, you will have to learn the following:

PHP or ASP, recomended PHP
MySql
Javascript
HTML
CSS
Plus an optional, FBJS, FBML, and XFBML, depending on your application setup.

If you want to make a flash game you will need to also learn these scripts
actionscript
coldfusion
and how to make flash animations.

Also, you will need to have a good understanding graphics, such as being a good photoshop user.
Or, you can spend on average a $300 dollar per hour fee for a graphics artist
^^ a graphics artist will provide amazing quality images, but are very expensive.

If your application is going to be complex, you will also want to learn
ajax and xml.
On top of that, you will want to have a good under standing in functions, algebra, formulas, and a decent understanding of algorithms.

There are also some other languages you can learn, but i wont list them to lessen your new headache.

I've been developing this app: http://apps.facebook.com/pewpewbangbang since September 16th, 2009, so as you can see it is quite a bit of work, especially when dealing with all of the frustrations that facebook give you.
