There may be multiple cases

1) Sometimes they are real, the image could be posted by relative or parents of child.

2) Sometimes real images are downloaded by like and +1 hungry people and they post such images on their profile to gain likes, shares & sympathy.

3) Sometimes such images were created by Photoshop and same process would be done, Begging for like, pray and share.

Now a days, 1) case is very rare, chances of 2) & 3) is very high so beware of such posts.

Sometime images are real and sometime fake.
So don't be emotional on looking that photo, just take him lightly, because only some images is real otherwise many photo are created by any software.
Hope you will understand.
