What exactly does the # sign mean in Twitter?
When someone types in #_____ with _____ being some phrase, what does that mean?

The "hash-tag" is a way of labelling a tweet.  You can search a particular hash-tag on Twitter, and see a feed of all tweets that have that tag.  So, it's a way of reading twitter by subject rather than by user.
