How do I know if my account have been hacked?

I think my yahoo account have been hacked, same goes to my facebook account. I really need to know this. I even have changed my passwords several times. Please, help me. Let me know something at least...

You know when youre hacked when your either;

1) Changed avatar.

2) Changed name.

3) Something has been writen on you profile(s).

4) Lastly, if any information has been changed at all.
