Instagram photos constantly failing, something wrong with Instagram?

I have an Instagram and an iPhone 5. Everytime I try to post an instagram photo, it fails even when I am on wi-fi or cellular service (4G). Sometimes when the photos don't fail, they post and I may get a few likes, but then the photo just disappears like I had never posted it or like I deleted it. Also sometimes when it posts the caption is blank even though I wrote something and when I refreshed the page my photo disappeared. This has been going on for a few weeks now. I know it isn't my internet because I go on the internet on my iPhone all the time. Could the problem be with the app? I closed all my other apps yet it still doesn't work. I also restarted my iPhone and still there was no fix. Should I re-install Instagram? Does this problem happen to any other people? Please help so I can go back to posting photos.
