on Facebook (FB), click on the Cog (around wheel with teeth) on the top right corner of your FB, and then select Settings, click Apps (on the left side), find the app on the main screen, then click the x next to Edit on the right, to remove it, it will pop up a box with confirmation, check the checkbox if you want to remove everything relates to the app, then click "Remove", that's it!
