What app did she use to make the background a picture ?

You'd need an image editor that supports layers and layer masks to make an edit like that.

Basically they have used a skull shaped layer mask, and overlaid an image and applied the skull layer mask to mask out the flowers, possibly putting multiple such flower layers, adjusting layer opacity or the blending mode.

You can use Adobe Photoshop (expensive), or the free image editor GIMP http://www.gimp.org
