Instagram account is private, but why..?

My instagram acccount is private. When i checked, I had this follower request from someone, but before I approved it, there was already a notification that this person had liked and commented on one of my photos. :/ I'm just worried about my account's security.

maybe you dont really understand the account settings and not everything you think is private is actually private
