I think like myspace way back when people are demoted or promoted from top friends or no longer top best friends ect people take it personally.

No. Because profile picture is about you, and not necessarily about your relations with someone else. You might have lots of friends who you would not want to trust with that information. Also, your profile can be probably viewed by anyone who searches for you on Google, including employers, contractors, enemies, acquaintances, the Big Brother, and what else not. You don't want to give away too much information about yourself in a publicly available element, and profile picture is generally one of those.
