On Twitter, What Does The "#" Mean In A Tweet?

So on Twitter people use the # in their Tweets. What does it mean and why do they use it?

i think that's how you trend a topic.

for example:
#nowplaying

people can click on that nowplaying text, and it'd lead them to a bunch of people who put #nowplaying in their tweets.
