I think that # means it would be a trending topic. People would use these in their tweets and the top ten trending topics are displayed somewhere on Twitter. For example, the band WTK once started a trending topic titled #hairwhips that made it all the way to number one! It was the most talked about topic all day!

it's mainly just @ and #.
# means its a topic..
like..
you can post a tweet sayin "#rememberwhen we were little?"
and if enough people copy the "#rememberwhen" in their tweets, it becomes a trending topic, as in, it makes it to the list on the right side of the home page..
