How do I get the "opinion" application out of my facebook profile?

I accidentally added the "Opinion" application( or whatever it is) on my facebook profile and I want to delete it. How am I supposed to do that? Please help me=)

On Facebook's main page, there is a box for your applications on the right side of the screen. Find the word "Edit", and click it.
That should take you to a list of all your applications. Just find the application you're trying to delete, and click "Remove".
