Where can I find a template or sample of a petition for visitation in the state of Indiana?

My boyfriend's ex-girlfriend has filed child support against him. The court date is set for late February. Before then he needs to submit a petition for visitation, otherwise visitation will not be addressed at that hearing. We cannot afford a lawyer, so the responsibility of writing this petition has fallen to me. I've found numerous sites on the information I need to include, I just need a format. Help?

They do not usually address visitation at a child support hearing, as it is considered a separate matter. These links will teach him what to be, and be doing for now. An attorney need cost a lot if he is prepared before seeing one.
