How can you remove an application on facebook?

I made the mistake of taking a friend's "how well do you know me quiz" on facebook. Now I receive notification every single time a friend makes a quiz, takes a quiz, comments on a quiz, etc. It's REALLY annoying. Does anyone know how to stop this? I clicked "block this application" on the quiz site, but no results so far.
p.s. I'd also like to be able to remove comments I've made on quizzes from my wall. Thanks!

Blocking prevents you from getting requests to add the application

You can either turn off the notifications for it, so you have the choice to still see people's quizzes when you want without getting notifications constantly or remove the application completely.

To turn off the notifications go to the notifications tab in your inbox and uncheck the box for how well do you know me

To remove it, go to the applications at the bottom and click edit applications. Then click the x next to how well do you know me

Step 1
Login to your Facebook account.
Step 2
Now in the upper left hand corner of the screen next to the word Application you will see a button with the word "edit." Click on that button.
Step 3
Once the application page is loaded, a list of all your applications can be found. Now simply press the button that says "Remove" and confirm.
