How to get rid of an app on Facebook?

There's this app on facebook and it keeps sending notifications to my friends that I tagged/commented them in a photo, but it's like a prank and instead it directs them to an app of a scary picture. It's sent the same notification to like half of my friends even though I didn't send it. IDK wtf it is, what do I do?

go to the bottom left, click on application then click on edit applications and just find what you want to block and click the 'x' button on the right
