How do I report someone I know has been trying to hack into my Facebook?

A few weeks ago, I set on my privacy settings on Facebook so that my account could be accessed from certain devices only. Tonight, I got a notification that someone had tried to logged in (or logged in successfully, not sure) from an unknown device. When I searched it, it was from a town way far from me, where the only person I know is someone who used to harass me.

Can I report this person (who also has an account) on Facebook to be blocked from the site or something? What can I do?

Go to settings and push Deactivate Account they will ask why then explain then.

You can go to their page and block them from being able to see you and you them. You do not KNOW this person done anything. No you cannot get them blocked from fb.
