How do I delete a fake Facebook account?

My phyco ex-boyfriend has set up a Facebook account in my name with a picture and everything. I don't have a Facebook account and I'm wondering how I delete the fake one my ex set up. I don't know the password or what e-mail address he used. I need help!

If you go onto the fake profile, on the left hand column, under the picture there should be an option to "Report/block this person".
Press that button then on the report option, select from the dropdown "fake profile" . Facebook will then take this into consideration and hopefully do something about it.
