It's called a "hashtag".

It's used within a message to identify a keyword or topic of interest and make it easier to do a search for it.

Clicking on a hashtagged word in any message shows you all other Tweets in that category.

Hashtags can occur anywhere in the Tweet.

Ok not a 100% sure but i think its used so that they can reference it to another conversation or link inside another twitter account....

Its a hash tag it is like when something well known is going on like say a episode of jersey shore everyone puts #jerzday or something somewhere on your page their should be trending topics and if a lot of people are #jerzday then it would be a TRENDING TOPIC and you click on it or search the hash tag to see what people are saying about the specific topic
