How to do the question and answer thing on facebook?

I was wondering how to do the thing on facebook when they ask you questions and you answer them. It's not social interview or yes no game. I know its under the notes application because that's what it says when I see that my friends have done it, but whenever I go my notes, it only shows the option of me writing one myself, not the question and answer thing. Please help me. Thanks.

Simply, copy the original note and paste the contents onto a new note. Erase their answers and put your own.
