How do I remove an application from facebook completely?

I'm trying to take off a game. Removing the app just blocks it, I want to delete it off my facebook for good and all my stats in the game as well.

if your a fan you have to first remove that and then you leave group or app

x on the right, then Remove
Here you can remove the application you want, but your stats and game points cannot be deleted.
