Facebook notification settings broken? Can it be fixed?

I play only one game (Sims Social), and it constantly posts to my wall and sends me notifications of people sending me stuff. I have turned off notifications under notification settings (the envelopes are grayed out for those notifications), and I still get all the notifications anyway. As far as I can tell, there was no change. Is there another way to turn off the notifications for the game and not let it post to my wall without removing the game?
The settings from the wall only allow to remove that individual post or remove the whole app. It would appear they've made it impossible to hide.

when somebody sends you a request click on the X and an option should come up to block the game click on that and you will see
Blocking (the game or app) will prevent others from sending you invitations and requests for this app and will prevent this app from getting any info about you. This will also prevent you from seeing (the game or app) if other people have it installed.
Just then press OK
