How to make a simple Facebook app?

I want to make a simple facebook application that shows the user's exam results I had this in mind:
Several drop-down lists where the user can choose what school subjects they have been studying,
and some radio buttons next to each drop down list so the user can choose from grade U to grade A.

Then it should show the subject names next to the grades they got on their profile.
Can anyone help me?
