It means that if u r following somone on twitter and u check off that u want to receive sms updates then everytime that individual updates there twitter account u will receive a text message saying whatever it is that person wrote in their tweet. And yes when u follow someone on twitter its free.

if you log into twitter.com with your name and password... go to settings.. mobile notifications... and turn it on... then you will get your friends tweets sent to your phone via a text (SMS)... do a @reply via text and send... and you just updated your twitter account... and since you have unlimited texting... this woulsd be great for uyou!
