Any criminal record anywhere of any kind.
Any record of violation of any visa regulations or immigration laws anywhere.
Not qualified by education, occupation, experience & expertise or other qualifications.
Any false statement, perjury, falsification, misstatement, forgery, etc, in any application, interview, supporting documentation, etc, connected with any visa or immigration application.
Not a legitimate, committed "till death do us part" marriage, marital relationship.

Those will do to start with. Canada is very strict!

Hundreds of things.

1. You don't meet the qualifications to apply. All categories has specific requires such as marriage to a Canadian, an approved job offer from a Canadian company in specific fields, you have $1.6M and an investment plan for $800k, you recently graduated from a Canadian university with certain degrees and found an appropriate job offer, you have a nomination from one of the provinces, you have a valid refugee claim, etc. If you don't meet the qualification, your application will be rejected.

2. You lied on your application form or didn't fill it out completely.

3. You have a criminal record, pending charges, immigration violations in other countries, or have ties with organized crime or terrorism. All applications for permanent residency must undergo background checks. If they are somehow unable to confirm your background.

4. You fail to provide documents or records as required in the application forms or as requested by CIC after wards or such documentation is not valid. Valid passports, marriage licenses, previous divorce agreements, labour market opinion reports for your job offer, proof of finances, educational records, etc..

5. You have a medical condition which are a danger to the safety of Canadians or would impose an undo burden on Canada's social or health care systems. All PR applicants must undergo medical exams.

6. You fail to pay the application fee.

7. You do not have sufficient money. Most categories require $13,000 in savings and no large outstanding debts. CIC may also assign higher amounts if you have dependents listed, do not have a job offer, are not employable, etc.

8. Your circumstances change during your application process. You are divorced or separated from your sponsor, your company withdraws its job offer, you commit certain crimes, etc.

9. Your spousal or employer sponsoring you has had any immigration violations.

10. Any dependent listed on your application form has any of the above issues. They fail a background checks, medical, etc. For minors with another parent, you require permission letters from your ex or a court order, etc.
