What does # mean? Why do I keep seeing it everywhere?
Why do I keep seeing that people use this on twitter and now facebook posts. Also on TLC channel it shows the # by whatever is showing. Does it mean the same thing in all of the cases I mentioned?

It's used to signify an afterthought on Facebook. For example:
Took the kids to the zoo today, had a great time bla bla. #lovinglife

It is a "hashtag" it is to categorize tweets to be more easily searched on twitter it is often made fun of by hipsters.

Yes it all means the same thing. Hashtag (#) is a twitter reference. It is the exact same thing a 'tagging' on facebook.
eg. Say you posted something like "chilling out watching friends on tv" you would then say something like #friends #GoodTime #IloveJoey or something like that.
Basicly if you understand tagging people/pages on facebook then # is the same thing on twitter Smile

Its called a hashtag.

On twitter, someone will put a # and then a word. Its kind of like adding tags to a youtube video or even categorizing a question on Yahoo Answers. Example, if someone posts a picture at DisneyLand, they'll hashtag the photo #DisneyLand and then if someone was to look up disney land on Twitter then their picture will come up.

well, '#' in its present form is called a 'hashtag' & in this form it was developed on Twitter. Everwhere else you see it is ment for you to check that hashtagged topic as it is trending on twitter. Facebook does not use it, but people like to cross use it it in a tongue in check way, but you to also to check twitter. FYI - Formally '#' was called a 'Pound Key' in reference to it on your phone & 'number' in all other instances like saying #1.
