You register yourself (free) then choose to follow interesting people or if you are a public figure then its a convenient means to keep your fans and press informed of you activities.

Its a waste of time for the average 'well adjusted' person because you gotta be pretty bored with life to want to keep track of every move that Lilly Allen etc makes.

Usually the novelty wears off pretty fast unless you are one of the many people who think they aught to be famous and try to compete with each other to gain followers, none of whom really give a #$% about anything you tweet (say)
