What does it take to get permanently blocked from sending friend requests on facebook?

I've been blocked from adding people for 30 days like 3 times, for no reason! I wasn't adding people that I don't know, I wasn't adding too many people in 1 day, and I'm using my profile picture, using a real name, and I never broke one of their rules. I usually cancel pending requests if they've got unanswered, and I still get blocked!! Most of them accept my request. So what does it take to get permanently blocked from sending friend requests? I've been hacked before and the hacker was adding strangers or people with mutual friends to my facebook, and it got me blocked 2-14 days, and I told facebook what happened, but they didn't listen! Please help?
I just need to know what does it take to get blocked forever
