where does the malware on facebook come from?

The Koobface work and the fake antivirus viruses on facebook, where do they come from?
Apps, fake people....?
But what about the other viruses like fake antivirus software. My friends computer had a fake antivirus from facebook, but he didnt say he got it from a link it just came.

Koobface is a worm. the worm sends a email with a link, this will be a youtube link this is where the malware comes from.
