How to begin an indie feature?

We will be filming the film partially in L.A. and the rest in Norway. My friend being a native from Norway has a few connections that could help us shoot there. These are my main concerns and questions:

How can we start a successful fundraising campaign for the film? (I know about sites such as kickstarter and using social media to generate hype, but are there any other ways?)

What are the preliminary legal proceedings during pre-production? More specifically, what should we do about insurance and contracts? What is necessary for an independent feature and more importantly what would be affordable?

What is the protocol to hire actors and crew?

How can we handle an international co-production?

Is it really necessary to establish our own production company? How do we go about that? Where do we start?
