friend confirmation on facebook but without sending a request?

I got a confirmation for friend on facebook from someone whom I do not know and never sent a request to. I am trying to figure out how this works. I did have people send me names they suggest I should be friend with. could the person get a suggestion and just confirm? I thought I need to send the request personally. Trying to reach someone on facebook but they don't get back. I'd like to understand this before sending the guy a message that I did not send any request! I think he is perhaps my friend's friend because I did see this name some where. could I accidentally hit something to trigger a request? thanks!
