It Signifies that you are using twitter and facebook at the same time.

It's called a hashtag, if you are using it in the content of someone posting something on Facebook, the hashtag can be used to denote more feeling into a certain word, like saying something to the effect of : I just walked into the room and saw my parents kissing #akward. The hashtag was used to promote the creepy feeling that the person had when they witnessed the action. Rather than trying to spell out A k w a r d! in a funny way like with spaces or capitalization, the hashtag brings an intensity to it.

The hashtag also can be used to help promote a certain trend or discussion around a certain common topic so that people can locate the discussion, group or topic ie #tornado to find out the latest of what has happened with victims of a tornado incident. Here's more information from WIKI:
