How to get his Facebook?

Okay, so I go to a guitar class. There's this boy there who is 2 years older than me. (Age isn't the main thing in this question). So basically our lessons are ending in 2-4 weeks and I really want to stay in touch with him.

The only way I can is by getting his Facebook. I only know his first name but we get on pretty well and talk, I just don't know how to ask him about adding me on Facebook. Any suggestions? I don't want to confront him, I'm quite shy.
