Change name of Facebook fan page with 200+ likes?

There used to be an option to request a change, but it's not there anymore. The FB Help Center says I can request a change, but it seems outdated by the new format. Anyone know how to do it now?

go to edit page and there will be a typing area for the name to change

I think it is depends on your fan page. If you want to change name of facebook fan page so you need to go the page name editing process. Otherwise you need to create new one. You need to share all thing your 200 likers. You can try the setting or fan page name edition.
