How to re-activate facebook account?

My friend had opened fb ac last yr. He had provided his gmail id to fb for notifications. Later he deactivated his gmail id. Also he forgot both i.e. gmail id & pswd. He also forgot his fb id & pswd.
Now he wants to de-activate his fb ac. For that I checked out his friend list on fb with the help of my fb ac. His name is removed from all his friend's list. He is afraid that somebody might have hacked his fb ac.
How can his fb ac be restored ?
