My friend and I can't add each other on Facebook?

I have a friend who lives in Ireland (I'm Canadian), and we used to be friends on Facebook. We talk on Skype and email almost every day, but about a year ago we got in a fight and he deleted me from Facebook and blocked me. Well now that we're talking again, we want to add each other back, but it's not working. We've both checked that we have each other unblocked, but every time I go to his profile there's no add button, and same when he goes to mine. I had my privacy settings fairly high because of people stalking me, but I lowered them so that everyone can find me, and it still doesn't work. We also tried messaging each other. Facebook said that he saw my message, but he said he didn't, then he sent me one and I didn't get it. Does anyone know how to fix this? Or how to contact Facebook staff for help? I know after we fought I freaked out and re-added him like 10,000 times, which he declined, but he should still be able to add me.
In response to JUSTIN: I thought it might be something like that, but how do I fix it? Or what can he do to fix it? I know I didn't choose that option, but he probably did, and even still shouldn't he be able to still add me?
