All I want to know has this happened to anyone else .... It Said I sent a friend request at this time and then they accept it but I never sent the request

Yes, it happens many times with many people, most probably it happens when you let an app use your account, when you open a app on Facebook for first time, it asks for your permission and all and at this step you need to be careful and read it properly which permissions it is asking for (I mean really) and not give full access permissions to the apps which are not from trusted developers, if you give full access permission to an not trusted developer's app (and there are plenty of them) they even get your account access like adding people without your awareness and sometimes even publish spam from your account without your knowledge, most of the time this is an attempt to make more fake subscribers or friends. For example if you send me a friend request and if I don't accept it then you might be shown publicly as subscriber of mine, so many people use this method for fake counts for subscribers. The only way is to delete the unknown people and spam posts manually and next time be careful when checking out new apps, you might also want to check your app center and edit settings of some apps which you may be suspicious of, also if you want to check if there is pending requests to unknown people, I will leave links below.
