How to report to Facebook about somebody publishing my email address to the public without my permission?
I am looking for a particular form. I did report but could not give a reason.

Actually, screenshot this, and tell them to take it down. If they say no, then call the police and show them your conversation, and them posting your email. They might get fined, or banned from FaceBook.
