How can I make a new Instagram account?

I completely forgot my username and password needed to sign in to Instagram and I don't know how to make a new account because I don't think there's anything which lets you do this. I didn't even remember to jot down the two details, how stupid of me.

I can't find my username.

If you are receiving a "No Account Found" message, or if the account that is returned is not yours, try locating your account through one of the following:
Search by your email address.
Find the URL for a photo you previously shared with friends, or on another social network like Twitter or Facebook. Usernames are listed at the top left of the photo page.

I can't remember my password.
If you can't remember the password to your account, or the password you are entering is not working, select "Forgot Password?" on the login screen.

New for Instagram v. 3.1, you can reset your password two different ways: a password reset email, or through your most recently connected Facebook account.
