Log into Facebook and click on the gear on the top right corner, then click 'Account Settings.'
Next, in the left pane, click 'Security.' Once there, click the blue text in the bottom of the main pane that reads, 'Deactivate your account.'

Edit: Just read Aaron's answer, and for the sake of your security, I wouldn't give him your email and password, because from that information, he could attempt a hijack of any other account you may own. Don't get me wrong, Aaron, you might be a perfectly harmless user, but there are more secure ways this problem can be solved.

then give your password and click submit. after 15 days your account will be deleted permanently from facebook.
