I seen a guy that will market to 50,000 twitter followers for only $5.

However, how do I know if said Twitter followers are "real" or not?

50,000 twitter followers for only $5? That's pretty exaggerated. Sure there are people out there capable of providing more then 100k followers in just one day but overall it costs a lot more.
Also the followers are usually just "real looking" and not really people.

If you want real people I suggest social media exchange websites like http://www.socialclerks.com because only real people can actually exchange followers on this kind of websites. It may be a lot slower and you won't get that much followers but overall you will get real people to follow you instead of some bots.
