How do you permanently block a facebook friend?

Go to your friend's page, and to the right of his picture, where you see "Friends | Message | Give Gift | Call", click the Friends option. Then just click the Report/Block button, block him or her and that's it.

you cannot block a friend, as when you block someone, they are removed from your friends list

perhaps you meant the restricted list

To block someone:

Click at the top right of any Facebook page.
Click How do I stop someone from bothering me?
Enter the name or email address of the person you want to block and click Block.
If you entered a name, select the specific person you want to block from the list that appears.
People will not be notified when you block them.
